Project Description Summary)
This project shall focus on adolescents and young people living with HIV (AYPHIV). This is because the adolescent HIV epidemic and particularly adolescent who are Lost to Follow (LTF) represents one of the challenging public health issues in Zambia. In the health facilities where we are working, the prevalence of Lost to Follow Up (LTFU) from ART is higher in adolescents than in adults. To strengthen HIV services delivery for adolescents, this project proposes to use differentiated service delivery (DSD) as an initiative to enhance service delivery benefits and improve retention to care for AYP. Improving comprehensive counseling services, follow-up for adverse reactions and introducing an ART outcome evaluation program may help reduce LTFU to an acceptable level. This project shall highlight the need to better understand the health-seeking behaviors of patients on ART and implement strategies in HIV clinics for better tracking services and minimizing LTFU from HIV care. Change in way of tracing services like community education, scaling up of health extension services community wide, and increasing awareness may reduce LTFU. Therefore, the project is now in need of a coordinator. )
Overall Purpose of the Role of the Coordinator)
The Project Coordinator will guide the overall implementation of the project within 12 months. He/she will be responsible for the overall management of the 3 project implementing sites, reporting to the management and critical stakeholders. He/she will take charge of community volunteers ensuring the project fulfils donor expectation. .

Main Duties/Expectations)
1. Hold orientation and regular planning meetings
2. Develop a common understanding and consensus on partnership and seamless project implementation with other community partners
3. Establish procedures that effectively utilize records to enhance LTFU tracing
4. Conduct review meetings with team and partners
5. Establishment of an adolescent network to enhance communication for the cross pollination of lessons and challenges and the Establishment of online interaction platforms and calendars (what’s app, face book, Twitter etc) to sustain online communication
6. Lead the team that will conduct defaulter tracing/follow up of clients that have been lost to follow up at health facility to enhance adherence and retention in care, strengthen alliance and enhance interface between the communities and facilities to bring LTFU back into care.
7. Lead the team that will conduct adherence counseling and psychosocial support through peer support and adolescent care clubs to improve adherence and retention and establish an effective system for data collection, analysis, and use of LTFU data between community to facility levels
8. Collaborate and strengthen community-based networks at community and at the site-levels to Implement evidence-based case finding approach in various targeted communities.
9. Conduct orientation in case finding approaches for service providers including CHW, with a strong focus on identifying adolescents.
10. Implement and sustain effective linkage procedures and systems for all newly identified PLHIV into care and treatment
11. Establish effective interface/reporting system between communities and facilities to ensure smooth hand-off of identified patients to facility and rapid treatment initiation
12. Forge strong relationships with the community to increase awareness and promote uptake of clinical services address/prevent further LTFU and support tracing and return to care
Other Assignments Specific to OYDC Centre
For OYDC Centre, your job will be to plan, budget, oversee and document all aspects of the activities at the centre. You will work closely with both M&E Manager and Project Manager to make sure that the scope and direction of the centre is on schedule and that the project is getting the necessary input from all support departments. You will coordinate all activities of the centre, from planning, implementation, to completion and working closely with centre clinician and M&E for evaluation.
KEY FUNCTIONS
1. To arrange, discuss and agree with program staff, service providers and other stakeholders on proposed activities, implementation and monitoring plan, roles and responsibilities of each team member.
2. Ensure that all grant required activities for the centre, indicators and objectives are successfully met within the project timeframe and allocated budget
3. Constantly work with the team to develop new and innovative ideas to promote the image of the centre
4. Work closely with the team to update project activity plans and discuss challenges/solutions
5. Schedule regular site visits and supervision where possible to ensure quality of services, accurate data collection and verify consumption rates of services.
6. Attend regular meetings (Mandevu clinic and Matero level 1 hospital) and any other coordination meetings
7. Produce timely reports for review

Qualifications and Experience
1. Diploma or higher qualifications in public health, project management, M&E or related
2. Experience in managing medium size public health projects is added advantage
3. Experience in working with adolescents will be a plus
4. Must be resident in Lusaka
