The Lusaka Provincial Health Office (LPHO) under the US President's Emergency Plan for AIDS Relief (PEPFAR) through a Cooperative Agreement (CoAg) Project with the Centers for Disease Control and Prevention (CDC) now invites qualified registered companies of goods, works and services to apply for suppliers' registration for the period 2023 to 2024.

ELIGIBILITY:-

Vendors should provide:-

1. Company Profile clearly indicating trading address and contact details;

2. Valid Tax Clearance certificate;

3. VAT Registration where a company is VAT registered. 4. Company registration certificate/PACRA registration; 5. Valid NAPSA certification

6. Filled and signed application form.

Submission Procedure

Interested companies should obtain or request for application forms without any applicable fee from Lusaka Provincial Health Office Procurement Unit between Monday to Friday from 08:00hrs to 15:00hrs on the address below:
