USAID Controlling HIV Epidemic for Key and Underserved Populations (CHEKUP II) Activity.

John Snow Health Zambia (herein referred to as “JSH”) is soliciting for suppliers for the USAID Controlling HIV Epidemic for Key and Underserved Populations (CHEKUP II) Activity. These procurements are funded by the United States Agency for International Development (USAID) and are subject to all applicable Federal Acquisition Regulations (FAR) and AID Acquisition Regulation (AIDAR).

The USAID Controlling HIV Epidemic for Key and Underserved Populations (CHEKUP II) Activity was awarded on September 13, 2022, with implementation through September 6, 2027. The Activity aims to support the Ministry of Health in mitigating against and attaining HIV epidemic control by increasing equitable access to, and use of person-centered, community-driven, and high-quality HIV prevention services for key and underserved (priority) populations. USAID CHEKUP II Activity operates in seven districts with the highest HIV-burden in Zambia. These include: Chirundu, Lusaka, Kabwe, Kapiri-Mposhi, Kasama, Chipata, and Nakonde.

REGISTRATION REQUIREMENTS

For initial registration, suppliers and bidders will be required to avail the following documentations to USAID Controlling HIV Epidemic for Key and Underserved Populations (CHEKUP II) Activity:

a. Name of company

b. Postal and Physical address

c. Phone Numbers

d. Email Address

e. Names of Directors and contact person(s)

f. Company Profile

g. Copy of Certificate of registration with relevant body where applicable

h. Copy of Zambia Revenue Authority Tax Registration Certificate, Tax Payers Identification Number (TPIN) and any other relevant documents.

i. Copy of Certificate of Incorporation or Registration, only companies registered in Zambia with the Patents and Companies Registration Agency (PACRA) and Registrar of Companies shall be eligible; and

j. Name(s) of banking details.

All interested local vendors should meet the regulatory requirements to undertake business in Zambia. Please note that submission of an EOI does not automatically qualify a vendor (s) to be included in the USAID Controlling HIV Epidemic for Key and Underserved Populations (CHEKUP II) Activity vendor database. Only shortlisted vendors that will meet the requirements will be contacted.

MODE OF SUBMISSION:

All completed applications must be marked clearly as “Registration of Suppliers for 2024”, the category being applied for.
