Job description:
• Follow procedures and cooperate with peers for best possible service delivery.
• Provide day to day support and supervision for patient referral /logistics team.
• Review patient cases at external referral hospitals including but not limited to treatment plans and patient length of stay.
• Identify treatment trends and patterns to manage clinical risk such as profile disease specific cases.
• Contribute to meeting departmental strategic objectives.
• Update client referral records / folders including but not limited to the patient registers.
• Ensure good quality outcomes are maintained regarding medical reports including ensuring medical records from the contracted health care providers are received timeously.
• Conduct monthly clinical audits on the referral documents, shared folder and patient registers to ensure quality standards are met.
• Track client information, schedules and medical reports in a confidential manner.
• Track missed appointments and initiate outreach and missed appointment policies to the relevant stakeholders.
• Liaise with the Health team coordinators to review and analyze data trends.
• Generate comprehensive clinical reports as required.
• Liaise with the health team to facilitate the mode of transportation for referrals and discharge patients back to site.
• Serve as a liaison between the contracted health care services, Emergency Medical Service workers, Emergency Care Officers and other providers for patient referrals/ transportation activations.
• Ensure that all SOP’s are updated and adhered too.
• Performs other related duties that may be required.
Qualifications
• Grade Twelve (12) Certificate or equivalent
• Diploma in Nursing or equivalent field
• HPCZ registration and current license
Experience
• Minimum 10-year experience in a similar role
• Qualified professional Allied health care professional/ or medical field
• High standard of computer literacy
• Ability to work with minimum supervision to achieve the established objectives
