3. Copy of Certificate of Registration/Incorporation.

4. Rate schedule for various services offered.

5. Physical and Postal Address of business.

6. E-mail address, telephone numbers

7. Copies of VAT certificate/a valid Tax clearance certificate.

8. Bank reference and bank details.
