JOB DETAILS:
Location: Choma District

Purpose:
To provide comprehensive ART treatment, care, retention and HIV prevention services to clients at the facility

Responsibilities:
• Undertakes directly or indirectly (through trained Counselors), to provide quality and cost-effective HIV Testing Services focusing on Provider Initiated Testing & Counseling (PITC) using prescribed screening tools, and Index Testing Services including Partner Notification Services (PNS).
• Ensure that all recipients of care are initiated on appropriate ARV regime
• Ensure timely change to appropriate ARV regime in the event of treatment failure or drug toxicity
• Ensure all Adverse Drug Reaction (ADRs) are documented and reported on time
• Ensure that all ROC have updated anthropometric measurements
• Ensure that all clinical and Laboratory forms are completely filled in
• Undertake and ensure adherence services are implemented at the facility
• Undertakes and ensures harmonization of clinical, Pharmacy and Laboratory appointments
• Undertakes to obtain/print/generate a list of ROC current on treatment who are due in week time for drug pick up, clinical visit, and viral load testing.
• Undertake and ensures the ROC on the list are reminded a week before, a day before and actual day
• Ensures that all ROC due for drug pick up, clinical visit, and viral load testing who did not show up are contacted/followed up within 24 hours.
• Ensures that all receipients of care defaulting treatment are actively followed up, their status determined, and brought back into care.
• Implement various DSD modalities such as 6/3MMD, Fast track, weekend & after hours, community post, scholars etc
• Promote good customer care in the provision of services to the ROC
• Undertakes and ensure Screening of OVC and linkage to OVC partners
• Ensures that a list of ROC due for VL in the coming week is generated and shared with the VL champion
• Ensures that all receipients of care due for Viral Load Testing have order and samples collected.
• Undertakes and ensures that all receipients of care on treatment for more than 6 months with a high viral load are enrolled and completes Enhanced Adherence Counseling (EAC).
• Ensures that all the VL results received should be entered into ROC file timely
Undertake and ensure VL champion actively follow up the VL results (lead timeline is two weeks)
• Ensure that all HIV positive persons eligible for TPT receive TB preventive treatment at the facility
• Ensure implementation of HIV prevention services such as PMTCT, VMMC, PrEP and PEP
• Support and promote the adolescents peer support groups activities, GBV and DREAMS (where applicable)
• Undertakes and ensure that all primary data collection tools (patient charts, registers, tally sheets, EHR) are appropriately completed.
• Provide VMMC services to eligible males in a safe and clean environment

Required Skills, Competencies and Experience
• Full Grade 12 Certificate with five (5) O' Levels including English,
• Mathematics & Science
• At least 3 years of working experience
• Diploma in Clinical Medicine
• An innovator, who can initiate new ideas and activities.
• Able to work with minimum supervision.
• Good Communication both oral and written.
• Job knowledge/technical expertise.
• Good Analytical skills.
