Submission requirements
A. Cover letter in the supplier's letterhead with company's date stamp and signature.
B. The name of the company, its address, telephone and fax numbers, email address and the details of the contact person.
C. Types of goods/services the supplier provides.
D. The number of continuous years in the supplying of medical equipments.
E. Name of the hospital/Organization where they supplied similar medical equipments.
F. Provide contactable reference documents of the supply of medical equipments.
G. Provide all certification/licenses of official registration of the company and any affiliation information of the company is also needed.
H. Provide the names of the owners/CEO/Managing Director
I. Any change to the company supplier needs to provide the information on the changes.
J. Financial information, latest certified bank statement should be provided.
K. Provide quality standard certificates.
Submission Procedure
Deadline for submission of bids is Thursday 8th December, 2022 at 14:00 Hours local time in the tender Box at Civic Centre. Submit hardcopies in a sealed envelope and marked supply, delivery installation and testing of medical equipment
