REQUIREMENTS

• Company Profile stating Company Directors and contact person.
• Mandatory Copy of VAT certificate. Tax clearance certificate and TPIN certificate.
• Copy of Certificate of Incorporation.
• Application should be sent with receipt of payment.

Submission Procedure

Application to be collected upon payment of non-refundable fee of K250.00, with Accounts department at Stimuli PR or deposite at the bank account number: 0142030000380 Indo Zambia Bank (Mandahill Branch), Branch code: 014. (Bank payments should provide proof of payment).
