Job Summary
The Tree of Life Head of Client Services is responsible for effectively working with the Administrative and Education Services Deputy Program Manager to manage the service’s caseload of clients ensuring that individuals’ cases are progressed and issues resolved. This person coordinates all relevant departments for various care-plan and guardian related meetings.
Dimensions of the Role
• Facilitate preparations of meetings in collaboration with all relevant departments for various client related meetings.
• Record all meeting minutes, discussions, and decisions, and communicate to relevant staff after meetings.
• Conduct training and onboarding of Care Plan Committee on new processes and systems.
Typical Responsibilities – Key End Results of Position
People Management
• Facilitate preparations of meetings in collaboration with all relevant departments for various care-plan related meetings.
• Attend regularly scheduled meetings with the supervisor.
• Ensure proper adherence to the policies and procedures of the Care Plan as stipulated by government officials, as well as the organization.
• Develop individualized care plans to address specific client needs including developing, implementing, monitoring, and evaluating the plan as required.
Communication & Reporting
• Apply specific workplace communication techniques to build and maintain relationships with clients and colleagues based on respect and trust.
• Record all meeting minutes, discussions, and decisions, and communicate to relevant staff after meetings.
• Communicate information on behalf of the Care Plan Committee to Tree of Life Administrative and Education Services Deputy Program Manager.
• Perform work within a legislative and ethical framework to ensure the provision of high-quality service delivery which supports the rights and interest of clients.
• Communicate effectively with clients and colleagues, including those from diverse cultural backgrounds and linguistic needs.
• Communicate to relevant staff and stakeholders when a child is admitted, is discharged, or is reintegrated back home; work in collaboration with Social Services.
Event Planning
• Coordinate the scheduling of Care Plan Intake Assessments & Reforecasts Calendar for the year.
• Maintain scheduling and venue procurement for all care-plan related meetings, such as Treatment Plan meetings, Reintegration Forecasting Meetings, Guardian Meetings etc.
• Schedule intervention meetings to monitor at risk children (behavioral, service adequacy etc.)
• Coordinate guardian meetings to ensure successful delivery of client services with relevant departments.
• Assist in preparation and/conduct field visits in collaboration with related FLMZ departments.
• Collaborate and participate in Social Welfare related activities in collaboration with Social Services.
Data Management
• Collect, organize, and maintain child Care Plan files.
• Maintain client information and case notes on the program database.
• Maintain up-to-date versions of all Care Plan documents and forms.
• Maintain the Care Plan Google Calendar and manage all event invitations and/or reminders for various Care Plan-related timeline events.
• Work with the Monitoring Evaluation and Research team to finalize the automation process for the already developed and implemented robust care plan system to effectively track and monitor the progress and milestones of 100% of children, ensuring timely interventions and personalized support for their growth and development, leading to successful reintegration.
The Employee shall also be expected to undertake any responsibilities as may be delegated by his or her supervisor.
